You will need to enter your phone number and your unique access Key provided to you by our office. You may preregister with our office by filling out our secure online Patient Registration Form. The security and privacy of your personal data is one of our primary concerns and we have taken every precaution to protect it. Please call our office at 843-766-0112 to find out your financial obligation that will be due the day of your appointment. Please remember to bring the referral form that was provided to you by your general dentist.
