Your Initial Consultation

Your initial appointment will consist of a consultation with Dr. Long explaining your diagnosis and treatment options. Occasionally, root canal therapy can be done the same day as the consultation. It is common in our practice to see emergency patients who present with discomfort or infection. When emergencies occur, we are able to accommodate patients as soon as possible, usually on the same day. However, a complex medical history or treatment plan may require an evaluation and a second appointment to provide treatment on another day.


Please provide the following information at the time of your consultation:

  • Your referral slip and X-rays if applicable
  • A list of medications you are presently taking
  • If you have dental insurance, please bring your insurance card and paperwork with you. This will save us time and allow us to help you process any claims. Click here for downloadable forms

IMPORTANT:

A parent or guardian must accompany all patients under the age of 18 at the consultation.

Please alert the office if you have a medical condition that may be of concern prior to surgery (e.g. diabetes, high blood pressure, artificial heart valves and joints, etc.) or if you are on any medication (e.g. heart medications, aspirin, anticoagulant therapy, etc.)

If your physician has advised you to take "antibiotic pre-medication" because of a heart murmur, hip, knee, or cardiac prostheses, please take the appropriate antibiotic as directed.
